摘要:糖尿病慢性創(chuàng)面因長(zhǎng)期炎癥反應(yīng)、細(xì)菌感染以及組織再生能力受損而難以愈合,仍是亟待解決的臨床問(wèn)題。本研究構(gòu)建了一種活性氧響應(yīng)型一氧化氮釋放水凝膠體系,以三羥甲基氨基甲烷改性的透明質(zhì)酸為基體,引入具有天然抗菌活性的 ε-聚賴(lài)氨酸,并結(jié)合功能化中空介孔雜化二氧化硅納米顆粒,實(shí)現(xiàn)藥物負(fù)載與刺激響應(yīng)釋放。該水凝膠可在溫和條件下快速凝膠化,形成穩(wěn)定三維網(wǎng)絡(luò)結(jié)構(gòu),表現(xiàn)出良好的組織黏附性能、抗菌活性及促愈合潛能。通過(guò)結(jié)構(gòu)與功能模塊的理性設(shè)計(jì),材料在氧化應(yīng)激條件下可實(shí)現(xiàn)異維A酸的可控釋放,并進(jìn)一步觸發(fā)按需 NO 生成。體內(nèi)外實(shí)驗(yàn)表明,異維A酸與 NO 的釋放均呈 ROS 水平依賴(lài)性且具可調(diào)控特征,從而顯著促進(jìn)創(chuàng)面閉合,增強(qiáng)膠原沉積與血管新生,并改善局部炎癥微環(huán)境。該水凝膠體系具有良好的生物相容性與智能響應(yīng)特性,提示其作為多功能創(chuàng)面敷料在糖尿病慢性創(chuàng)面高效治療中具有潛在應(yīng)用價(jià)值。
